FHC Injury Process

 

1.               Please ensure the injury has been reported on the score sheet.

2.               Please fill in the FHC Injury Report Form and leave in the FHC post box.

i.      Please feel free to contact any of the Injury Officers of our Club for further information

3.               Please follow the Hockey Victoria Insurance Personal Accident Claims Procedure 2011-2012 (you have 180 days from the date of the injury to lodge a claim)
